Fill out form, specifying the paper title, authors, and tracking code (the name of your deposited file), and sign it (one author's signature is sufficient).
Please fax the completed form to the Professional Book Center at +1 (305) 946-8106 as soon as possible.
The instructions on the form state that you must do this no later than Tuesday, April 4, the same day as your revised paper is due. However, since we forgot to send out a notice to that effect, please fax it by Friday, April 7.
